New Horizons Home Care, Inc., a locally owned and operated home care agency, provides in-home care and case management services to NC Innovations participants and potential recipients.  At New Horizons Home Care, Inc., our case managers coordinate state, local, and community resources to best fit the needs of each consumer.

Our talented team has many years of experience in case management.

At New Horizons Home Care, Inc., we feel very strongly that each client is a unique individual with specialized needs.  Our caring and professional staff will seek to protect the rights of our clients and to serve their needs to the fullest extent possible.  It is our goal to make sure that your needs and concerns are not only acknowledged, but that the services that best meet those needs are put into place both timely and effectively.  To further serve our customers, our staff is trained in many aspects of client care including nursing and social areas.  Our case managers and support staff are educated and trained according to the most stringent federal and local guidelines.

We believe that the client has the right to choose the best provider of services to help meet their need. We also contend that the client has the right to voice their concerns and be an active part of the decision-making team.  We promote integrity throughout our organization.  We encourage our case managers to be honest and upfront with you, the consumer, throughout the attainment process and also through any ongoing changes that affect NC Innovations consumers.
